Welcome to the Nurse's Office,
The health room is open each day from 8 AM until 2:45 PM. Students coming to the nurse must bring their agenda books so we can sign the passport section. This record keeping procedure will track how many times a student visits and assure staff that the students are where they should be.
Scoliosis or curvature of the spine is screened on all 6th grade female students and all 8th grade male students, with the exception of those students whose parents/guardians contact the nurse to decline the screening. Parents will be notified by mail if their child fails a screening.
Students in 6th grade are required to have a new physical on file before entering 7th grade. In addition, they are also required to have a Tdap booster, and Meningococcal (MCV) vaccine to enter 7th grade. The immunization requirements for the state of Connecticut are outlined on page 3 of the Annual Health Assessment Record form, which can be found for download on this page.
Students with any medication at school must have a new order each year from their provider. The medication authorization form to be filled out by the provider is available for download on this page or can be obtained from the nurses office. If your child does have medication in the health room, you will receive notification to pick it up at the end of the year. Any medication not picked up by the last day of school will be destroyed and disposed of.
The health room is able to provide Acetaminophen or Ibuprofen to students whose parents/guardians have filled out the appropriate form available in the health room. Permission slips for Tylenol and Ibuprofen will be sent home the first week of school.
Students who are signed up may go to the School Based Health Clinic but must always begin in the health room where a referral note will be sent to the Nurse Practitioner
