Registration of all school age children in grades K-12 takes place at: Central Office, 1300 Flanders Road, Mystic, CT 06355 Please call 860.572.2197 or email to make a registration appointment.  

In order to complete the registration process, the school district will need proof of age (e.g. birth certificate, Passport, etc.), proof of residency and updated medical records for the child or children you wish to register. 

Confidential student information related to your child’s registration cannot be sent via email. Please remit these documents by one of the following methods below.

What to bring WITH You for your appointment

Before you leave home, take a moment to go through the following checklist to make sure you bring all the appropriate documents and evidence needed to complete the registration process.

  • Proof of residency -  Policy P 5112.2 Admission Requirements for Resident Students outlines acceptable proof of residency documentation.

  • Picture ID - Driver's license, military id, passport, etc.

  • Proof of Child's Age - Hospital, Physician or Religious Certificate showing date of birth; Adoption Record, Birth Certificate, Previously verified school records.

  • Proof of guardianship, if you are not the birth parent or if you are divorced - Adoption papers, court order clarifying guardianship, guardianship papers from Probate Court, DCF form 603 (must be signed by DCF or legal designee)

  • Child's Immunization Record - Per Department of Public Health, proof of immunizations is required 

  • Physical Exam - Proof of a physical exam is required to register . The health assessment form side one must be completed by the parent; side two is to be completed and signed by a health care professional.
