STEP 2:

 

Provide the Required Documents to Complete Your Child's Registration

See the document checklist below. Your child's registration is not complete until all required documents are provided. Appointments are held at our offices located at 1300 Flanders Road, Mystic, CT, 06355.


 

Registration Document Checklist:

  • Your Picture ID - Driver's license, military id, or passport
  • Proof of Your Child's Age - Hospital, Physician or Religious Certificate showing date of birth; Adoption Record, Birth Certificate, Previously verified school records.
  • Proof of guardianship, if you are not the birth parent or if you are divorced - Adoption papers, court order clarifying guardianship, guardianship papers from Probate Court, DCF form 603 (must be signed by DCF or legal designee)
  • Child's Immunization Record - Per Department of Public Health, proof of immunizations is required 
  • Physical Exam - Proof of a physical exam is required to register . The health assessment form side one must be completed by the parent; side two is to be completed and signed by a health care professional.
